Minimum Import File Format should be as below

Client last name , first name , middle name , telephone number , appointment date , start time , appointment subtype , patient's language

If file separator is part of any data value, please enclose that value in double quotes.
E.g. if file separator is comma and if address is 1687, Old Henderson road then it should be written in csv file as "1687, Old Henderson road".

Compulsory fields are -

Client last name , first name , telephone number , appointment date , start time

Your file format will be -

Client last name , first name , , telephone number , appointment date , start time , , ,

Date separators could be / or -.

Time format hh:mm AM/PM for 12 hour and hh:mm for 24 hour, separators could be : or .

If appointment sub type are defined, then it must be specified.

Language code 2 character code used by Windows OS. E.g. English=EN, Spanish=ES.

Only those languages can be imported that are selected by the provider on Set Preferences Page If no language is specified then it will select provider's default language.
